The Kansas Patient Centered Medical Home Initiative (PCMH) Initiative is a project intended to improve health care in Kansas through advancing the PCMH health care model. The initiative is an effort of the Kansas Academy of Family Physicians in conjunction with the Kansas Association of Osteopathic Medicine, the Kansas Chapter of American Academy of Pediatrics, the Kansas Chapter of the American College of Physicians, and the Kansas Medical Society.

The initiative provides education and information regarding the Patient Centered Medical Home (PCMH) and encourages practices to move to the PCMH model thereby improving population health and clinical outcomes. Additionally the purpose of the initiative is to:

Provide education about the Patient Centered Medical Home to medical providers
Offer information to the public about the PCMH
Provide facilitation for eight physician-led practices across the state, transforming to a PCMH
